Medication Administration form

Medication Administration forms are required for the nurse to administer ANY medication including prescription and over the counter medications. The forms must be downloaded, filled out by the child's physician and signed on the back by the parent in order to be valid. Please return the completed form along with the medication to the school nurse. A parent or guardian must drop off the medication to the nurse. A student may be permitted to self-administer medciation for asthma or other potentially life threatening illnesses. A certification in writing from the child's physician , must be recieved stating the that student is capable of and has been instructed in the proper administration of the required medication.
